Rights of the patient include, but are not limited to:
(1) The exercise of these rights without discrimination as to sex, ethnic origin, cultural differences, religious background, or economic situation.
(2) Treatment by all health care personal in a polite, respectful, and professional manner.
(3) Care as promptly as possible.
(4) Information as to diagnosis, treatment, alternatives of treatment, and risks and complications.
(5) Participation in all decisions concerning care and treatment.
(6) Confidently of medical records.
(7) A copy of medical records at any reasonable time.
(8) Being informed of the cost of services.
(9) The right to seek a second opinion.
(10) The right to have another physician take over medical care and treatment at any time.
(11) The right to refuse any service or study.
The patient has responsibilities to health care personnel, which include but are not limited to:
(1) Provide accurate information on all forms.
(2) Provide complete health history as accurately as possible.
(3) To be courteous to health care personnel.
(4) To follow all instructions for care and if not, to inform the office.
(5) To fulfill requests for tests, physicals etc. requested by this office promptly.
(6) To have a responsible adult driver for postoperative transportation as requested.
(7) To cancel office appointments by phone, please allow a minimum of 24 hours notice, in advance or greater if possible. Surgical appointment cancellations require a minimum of 15 days or more prior to procedure date.
(12) The right to refuse any research study.
